Ophthalmology Consultation

During the consultation, the ophthalmologist will ask you about your symptoms and medical history. Then, they will perform a series of basic tests in their office: they will measure your visual acuity by having you read letters at a distance, they will take your eye pressure (using a small puff of air or light contact) and they will examine the structures of your eye (cornea, lens, retina) using a special microscope called a slit lamp. If they detect anything unusual, they will explain your diagnosis and advise you if you need additional tests or any drop treatment.

Ocular biometry measures the exact anatomical dimensions of the eyeball, highlighting axial length (distance from the cornea to the retina) and corneal curvature. These parameters are fed into complex mathematical formulas to calculate the exact diopter of the artificial lens (IOL) that will replace the opaque lens during cataract surgery or a clear lens.

Pachymetry measures corneal thickness in microns. This information is crucial for two reasons: first, in laser refractive surgery, the surgeon needs to know if there is enough corneal tissue to reshape and correct myopia. Second, very thin corneas can give falsely low intraocular pressure readings (masking glaucoma), while thick corneas can give falsely high readings.

Corneal topography is a non-invasive diagnostic technique that maps the curvature, thickness, and relief of the cornea (the transparent outer lens of the eye). It is vital for detecting corneal irregularities, diagnosing degenerative pathologies such as keratoconus, and is a mandatory prerequisite for evaluating whether a patient is a candidate for refractive surgery (laser or intraocular lens).

Aqueous humor (intraocular fluid) drains through a structure called the iridocorneal angle (where the iris and cornea meet). Gonioscopy allows direct visualization of this angle to determine if it is open (open-angle glaucoma) or closed/narrow (angle-closure glaucoma). This distinction is critical, as the medical or surgical treatment varies completely.

Computerized campimetry (or perimetry) evaluates retinal sensitivity and the condition of the optic nerve in its entirety, not just central vision. Silent diseases such as glaucoma first damage lateral (peripheral) vision. This test allows the detection of scotomas (blind spots) very early, long before the patient notices them in their daily life.

Retinography is a diagnostic technique that allows for colored photographs of the deeper structures of the eyeball: the retina, the macula (responsible for detailed central vision), and the optic nerve disc. It is a fundamental tool for screening and monitoring silent diseases such as diabetic retinopathy, age-related macular degeneration (AMD), and glaucoma.

Optical Coherence Tomography (OCT) uses light waves to take cross-sectional images (optical slices) of the retina and optic nerve. It works similarly to an ultrasound, but uses light instead of sound, which provides much higher resolution. It allows for measuring the thickness of the retinal layers down to the millimeter, being key for diagnosing macular edema or glaucoma damage.

Ocular ultrasound uses high-frequency sound waves (ultrasound) to generate two-dimensional structural images of the inside of the eye and orbit (B-mode). It is essential for diagnosing retinal detachments, vitreous hemorrhages, lens dislocations, or the presence of ocular tumors when opacities of the transparent media of the eye prevent the use of a slit lamp or ophthalmoscope.

Eyestrain (asthenopia) is a very common symptomatic condition today due to the prolonged use of digital screens (Computer Vision Syndrome). Its main causes include poorly corrected refractive errors, convergence insufficiency (difficulty keeping the eyes focused inward at close distances), accommodation problems (focusing), and blinking alterations that cause dry eyes.

YAG laser peripheral iridotomy is the first-line treatment for preventing or treating angle-closure (or narrow-angle) glaucoma. This condition occurs when the fluid inside the eye (aqueous humor) cannot drain properly, causing a dangerous increase in intraocular pressure that can damage the optic nerve. The laser resolves this anatomical blockage by creating a new drainage pathway.
